PLAYER REGISTRATION: (all players must be registered and forms submitted before paricipating in soccer programs)

To register a player, the followiing forms must be submitted after online registration:

1. Copy of the player’s birth certificate (returning player should have a BC on file)
2. Medical Release - (for players on teams that will be traveling to play in Phoenix or out of state the form must be notarized)

COACH AND MANAGER REGISTRATION: (all forms must be submitted before working directly with players)

1. Online Coach / Manager Application 
2. AYSA Volunteer Disclosure.  Once the online application is submitted, you will receive an email from the AYSA with a link to an online disclosure form.
